Overview

• Provide social work services in an acute hospital setting, addressing patient needs and ensuring quality care.

Key Responsibilities & Duties

• Conduct patient assessments to identify social, emotional, and financial needs.
• Develop and implement individualized care plans in collaboration with medical teams.
• Provide counseling and support to patients and their families.
• Coordinate with community resources to ensure continuity of care post-discharge.
• Maintain accurate and timely documentation of social work activities.
• Participate in interdisciplinary team meetings to discuss patient progress.
• Educate patients and families about available resources and support systems.
• Ensure compliance with ethical standards and confidentiality regulations.

Job Requirements

• Master of Social Work degree from an accredited institution.
• Minimum of 2 years of experience, preferably in an acute hospital setting.
• Initial social work licensure as required by state law.
• Strong interpersonal and written communication skills.
• Demonstrated analytical and problem-solving abilities.
• Ability to manage multiple tasks effectively in a fast-paced environment.
• Proficiency in providing excellent customer service.
• Capability to work independently with high attention to detail.
