Job Description

Overview

  • Provide clinical supervision and support to teams within a residential treatment facility for youth aged 12-18.
  • Develop and implement transitional and permanency programs to ensure successful community integration for residents.
  • Collaborate with interdisciplinary teams to address residents' educational, emotional, and social needs effectively.
  • Ensure compliance with regulations and guidelines while maintaining high standards of care and documentation.
  • Oversee discharge planning and family engagement to support positive outcomes post-treatment.
  • Participate in clinical and treatment meetings to enhance program effectiveness and resident care.
  • Maintain collaborative relationships with local resources and agencies to support residents and families.
  • Support staff development through training in therapeutic crisis intervention and verbal de-escalation techniques.

Key Responsibilities & Duties

  • Supervise transitional coordinators, permanency-family connections specialists, and intake specialists within the residential program.
  • Assess and address team competencies and compliance with documentation standards through chart reviews and community visits.
  • Coordinate community-based supports and resources for residents and families to ensure successful transitions.
  • Lead meetings with community-based programs to enhance family support services and resource accessibility.
  • Monitor utilization of transitional funds in compliance with policies to support resident needs.
  • Provide direct transitional coordinator services as needed to ensure program continuity.
  • Participate in administrative meetings to present updates and enhance program strategies.
  • Ensure compliance with OMH regulations and guidelines while maintaining high-quality care standards.

Job Requirements

  • Master's degree in Social Work or related human services field required.
  • Minimum of 2 years of experience providing direct services to children with emotional disturbances.
  • Supervisory or managerial experience in the social services sector is preferred.
  • Experience in residential settings and familiarity with therapeutic crisis intervention techniques.
  • Strong communication and documentation skills to ensure compliance and effective team collaboration.
  • Willingness to travel to families' homes and work one weekend day per week.
  • Ability to align with the mission statement and contribute to program enhancement.
  • Proficiency in electronic medical records systems is highly desirable.
