Job Description

Overview

  • Provide comprehensive medical care to adult patients, including evaluation, management, and treatment of acute and chronic conditions.
  • Engage in health promotion activities and patient recruitment efforts to enhance community health outcomes.
  • Collaborate with a multidisciplinary team to deliver quality clinical care and participate in peer reviews and quality improvement projects.
  • Maintain accurate medical records, complete EMR documentation, and ensure compliance with applicable laws and regulations.
  • Participate in pre-visit planning activities and attend clinic, provider, and staff meetings regularly.
  • Provide initial assessment and management of mental health conditions, prescribing medications as necessary.
  • Advise patients on treatment recommendations, lifestyle modifications, and preventive screenings.
  • Comfortably work with diverse populations, including recent immigrants, ensuring culturally competent care.

Key Responsibilities & Duties

  • Evaluate and manage acute and chronic medical conditions using evidence-based practices.
  • Order and analyze diagnostic tests to inform clinical decision-making and prescribe appropriate treatments.
  • Perform mental health screenings and prescribe medications for conditions such as depression and anxiety.
  • Engage in health promotion activities and community outreach to improve patient recruitment.
  • Maintain detailed medical records and ensure timely completion of EMR documentation and billing.
  • Collaborate with other healthcare professionals to provide integrated care to patients.
  • Participate in certification and continuing education activities to maintain professional competency.
  • Comply with HIPAA regulations and other laws applicable to clinical practice.

Job Requirements

  • Master's Degree in Nursing (MSN) or Doctoral Degree in Nursing Practice (DNP).
  • Certification as a Family Nurse Practitioner or Adult Nurse Practitioner.
  • Current and valid New York State license and DEA certificate.
  • Updated BLS and infection control certifications.
  • Proficiency in Spanish or an Asian language is preferred.
  • Experience with PrEP/PEP, STI testing and treatment, and mental health screenings.
  • Familiarity with eClinicalWorks EMR system is advantageous.
  • AAHIVM certification and/or LGBT health certification is a plus.
