Job Description

Overview

  • Provide comprehensive medical care to adult patients, focusing on acute and chronic conditions.
  • Conduct initial assessments and manage mental health conditions such as depression and anxiety.
  • Order and analyze diagnostic tests using evidence-based practices to guide clinical decisions.
  • Prescribe treatments and medications within the scope of licensure and evidence-based guidelines.
  • Advise patients on preventive screenings, lifestyle modifications, and treatment plans.
  • Collaborate with a multidisciplinary team to ensure quality care delivery.
  • Participate in health promotion activities and recruitment efforts for the clinic.
  • Maintain accurate medical records and complete documentation in a timely manner.
  • Engage in quality improvement projects and peer review processes.

Key Responsibilities & Duties

  • Evaluate and manage acute and chronic medical conditions for adult patients.
  • Perform mental health screenings and prescribe medications for conditions like depression and anxiety.
  • Order diagnostic tests and utilize evidence-based methods for clinical decision-making.
  • Prescribe treatments and medications, ensuring adherence to licensure scope and evidence-based practices.
  • Advise patients on treatment plans, lifestyle changes, and preventive screenings.
  • Collaborate with healthcare teams to provide holistic patient care.
  • Participate in clinic meetings and pre-visit planning activities.
  • Engage in quality improvement initiatives and maintain certifications.
  • Ensure compliance with healthcare laws and regulations, including HIPAA.

Job Requirements

  • Bachelor of Science in Nursing (BSN) with a Master's or Doctoral degree in Nursing.
  • Certification as a Family Nurse Practitioner or Adult Nurse Practitioner.
  • Current and valid NYS license and DEA certification.
  • Updated BLS and infection control certificates.
  • Proficiency in Spanish or an Asian language is preferred.
  • Experience with PrEP/PEP, STI treatment, and mental health screenings.
  • Familiarity with eClinicalWorks electronic medical records system.
  • AAHIVM certification and/or LGBT health certification is a plus.
  • Comfort working with diverse populations, including recent immigrants.
