Job Description

Overview

  • Provide comprehensive care to families, focusing on health promotion and management of acute and chronic conditions.
  • Engage in health promotion activities and recruitment efforts for the healthcare site.
  • Collaborate with multidisciplinary teams to ensure quality patient care and effective treatment plans.
  • Utilize eClinicalWorks for accurate documentation and patient record management.
  • Participate in quality improvement projects and peer review processes to enhance clinical outcomes.
  • Provide mental health screenings and prescribe medications for conditions like depression and anxiety.
  • Prescribe evidence-based treatments, including medications, within the scope of licensure.
  • Comfortably work with diverse populations, including recent immigrants, ensuring culturally competent care.
  • Maintain updated certifications and comply with all applicable healthcare regulations.

Key Responsibilities & Duties

  • Evaluate and manage acute and chronic medical conditions for adult patients.
  • Perform initial assessments and management of mental health conditions.
  • Order and analyze diagnostic tests using evidence-based principles.
  • Advise patients on treatment recommendations and preventive screenings.
  • Document patient interactions and treatment plans in the electronic medical record system.
  • Collaborate with healthcare teams to ensure comprehensive care delivery.
  • Participate in clinic meetings and pre-visit planning activities.
  • Engage in quality improvement initiatives to enhance patient care.
  • Provide culturally sensitive care to diverse patient populations.

Job Requirements

  • Master's Degree in Nursing (MSN) or Doctoral Degree in Nursing Practice (DNP).
  • Certification as a Family Nurse Practitioner or Adult Nurse Practitioner.
  • Current and valid NYS license and DEA certification.
  • Updated BLS and infection control certificates.
  • Proficiency in Spanish or an Asian language preferred.
  • Experience with PrEP/PEP, STI treatment, and mental health medication prescription.
  • Familiarity with eClinicalWorks and ambulatory care settings.
  • AAHIVM certification and/or LGBT health certification is a plus.
  • Minimum two years of experience as a Nurse Practitioner required.
