Overview

Key Responsibilities & Duties

• Coordinate patient care plans, ensuring seamless transitions and optimal resource utilization.
• Monitor and document patient progress, providing timely updates to healthcare teams.
• Collaborate with physicians, nurses, and other professionals to deliver comprehensive care.
• Educate patients and families on care plans, fostering understanding and compliance.
• Ensure adherence to hospital policies, standards, and regulatory requirements.
• Participate in quality improvement initiatives to enhance patient care services.
• Maintain accurate and thorough documentation of all case management activities.

Job Requirements

• Associate of Science (AS) in Nursing or equivalent educational background required.
• Minimum of 2 years of professional nursing experience preferred.
• Active RN license in good standing with the relevant state board.
• Proficiency in case management, patient care coordination, and healthcare protocols.
• Strong communication and interpersonal skills for effective collaboration.
• Ability to thrive in a fast-paced, on-site hospital environment.
• Experience in similar healthcare assignments is advantageous.
