Overview

Key Responsibilities & Duties

• Deliver high-quality case management nursing care to patients during assigned shifts.
• Monitor patient conditions, document observations, and adjust care plans accordingly.
• Coordinate with healthcare professionals to develop and implement effective care strategies.
• Educate patients and families on health management and recovery processes.
• Ensure compliance with hospital policies, procedures, and regulatory standards.

Job Requirements

• Associate of Science (AS) degree in Nursing or equivalent qualification.
• Minimum of 2 years of nursing experience in a clinical setting.
• Active Registered Nurse (RN) license in good standing.
• Proficiency in case management and clinical documentation practices.
• Strong interpersonal and communication skills for patient and team interactions.
