Job Description

Overview

  • Provide compassionate medical care to hospice patients, ensuring adherence to care standards and regulations.
  • Collaborate with interdisciplinary teams to develop personalized care plans for patients and families.
  • Offer expertise in symptom management and terminal prognosis evaluation to enhance patient comfort.
  • Conduct home visits to provide direct patient care and support to families.
  • Ensure seamless transitions between care settings, maintaining continuity of care.
  • Participate in educational initiatives to promote best practices in hospice care.
  • Lead efforts to optimize hospice utilization and improve service delivery.
  • Provide leadership and guidance to enhance the quality of hospice programs.

Key Responsibilities & Duties

  • Evaluate and certify terminal prognosis, ensuring compliance with hospice guidelines.
  • Collaborate with attending physicians to coordinate comprehensive patient care plans.
  • Document patient care services accurately in electronic medical records systems.
  • Lead interdisciplinary team meetings, fostering collaboration and effective communication.
  • Provide medical expertise for pre-admission assessments and symptom management strategies.
  • Support patients and families through sensitive discussions about care options and prognosis.
  • Review medication regimens to ensure appropriateness and address adverse reactions.
  • Participate in quality improvement initiatives to enhance hospice care services.

Job Requirements

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
  • Board Certified or Board Eligible in Hospice and Palliative Medicine preferred.
  • Current and unrestricted medical license in the applicable state required.
  • DEA license and prescriptive authority as per regulations mandatory.
  • Minimum of 1 year of experience in hospice or palliative care preferred.
  • Proficiency in electronic medical records systems and documentation standards.
  • Valid driver’s license and reliable transportation for community visits required.
