Key Responsibilities & Duties

• Engage with members regularly to review and update their care plans, ensuring alignment with their evolving needs.
• Coordinate care services with families, healthcare providers, and vendor agencies to optimize outcomes.
• Document all member interactions and updates promptly in compliance with organizational standards.
• Participate in team meetings to discuss member progress and strategize care improvements.

• Provide bilingual support to facilitate effective communication with Bengali-speaking members.
• Utilize Excel for documentation and reporting tasks, ensuring accuracy and efficiency.

Job Requirements

• Bachelor's degree in a relevant field or equivalent professional experience.
• Minimum of 1 year of experience in Managed Long-Term Care (MLTC) or similar settings.
• Proficiency in Bengali language to support effective communication with members.
• Basic Excel skills for managing documentation and performing data-related tasks.
• Strong understanding of member assessment and clinical review processes.
• Ability to work remotely while maintaining high levels of productivity and member engagement.
• Excellent communication skills to interact effectively with diverse populations.
• Familiarity with care management documentation standards and reporting requirements.
