Overview
- Coordinate and oversee hospice care services within inpatient settings, ensuring compassionate and effective delivery of care.
- Develop collaborative relationships with hospital teams to support seamless hospice admissions and transitions.
- Conduct comprehensive patient assessments to determine hospice eligibility and create personalized care plans.
- Educate patients, families, and healthcare providers about hospice services and benefits to foster understanding and trust.
- Collaborate with interdisciplinary teams to ensure high-quality care and compliance with regulatory standards.
- Provide specialized clinical expertise to support patients receiving General Inpatient (GIP) level of care.
- Adapt to dynamic healthcare environments to address referral needs and promote hospice services effectively.

Key Responsibilities & Duties
- Strengthen partnerships with hospital teams to facilitate hospice care coordination and admissions.
- Respond to referrals promptly, ensuring accurate documentation and adherence to regulatory guidelines.
- Evaluate patients’ clinical information to determine hospice eligibility and appropriate care plans.
- Serve as the RN member of the Inpatient Hospice Interdisciplinary Team (IDT), contributing to collaborative care planning.
- Coordinate daily care for patients receiving General Inpatient hospice services, ensuring optimal care delivery.
- Educate and support patients, families, and hospital personnel regarding hospice care processes and benefits.
- Maintain detailed and accurate documentation of patient care activities and referral processes.
- Collaborate with healthcare providers to ensure seamless transitions and continuity of care for hospice patients.
Job Requirements
- Associate of Applied Science (AAS) in Nursing or related field required.
- Minimum of 2 years’ experience in Medical-Surgical or Community Health nursing.
- Preferred 4 years of experience in Hospice or Oncology nursing.
- Active RN licensure in New York State and CPR-BLS certification required.
- Valid NYS Driver's license preferred for travel between hospital locations.
- Proficiency in Microsoft Office applications and strong communication skills.
- Ability to work flexible hours, including weekends, to meet position needs.
- Excellent organizational skills and capability to collaborate within interdisciplinary teams.
