Overview

- Oversee discharge planning processes, ensuring compliance with healthcare regulations and organizational standards.

- Develop and maintain strategic relationships with physicians and post-acute care providers.

Key Responsibilities & Duties
- Direct departmental operations, including staffing, onboarding, and competency development initiatives.
- Ensure adherence to care coordination and discharge planning processes across all cases.
- Monitor departmental performance using data analysis and implement sustainable improvement strategies.
- Collaborate with financial and operational leaders to align departmental goals with organizational objectives.
- Establish and maintain relationships with clinical and ancillary service leaders.
- Manage on-call rotations for weekends based on hospital volume and operational needs.
- Uphold the organization’s Code of Conduct and Mission Statement in all professional activities.
Job Requirements
- Bachelor of Science in Nursing (BSN) is required; Master’s degree is preferred.
- Active Registered Nurse (RN) licensure or equivalent social work licensure is required.
- Minimum of 3 years of acute care hospital case management experience is required.
- At least 2 years of leadership experience in case management is required.
- Proven ability to build strategic relationships with physicians and post-acute care stakeholders.
- Excellent communication, presentation, and motivational skills are essential.
- Strong adaptability and ability to manage multiple priorities effectively.
- Self-motivated with a robust work ethic and ability to develop actionable plans independently.
