Job Description

Overview

  • Lead the case management department, driving strategic initiatives to enhance patient care and operational efficiency.
  • Oversee discharge planning processes, ensuring compliance with healthcare regulations and organizational standards.
  • Collaborate with interdisciplinary teams to optimize resource utilization and improve care outcomes.
  • Develop and maintain strategic relationships with physicians and post-acute care providers.
  • Provide leadership and mentorship to case management staff, fostering professional growth and team cohesion.
  • Adapt to dynamic healthcare environments, managing multiple priorities effectively.
  • Report to executive leadership, contributing to organizational strategy and decision-making.

Key Responsibilities & Duties

  • Direct departmental operations, including staffing, onboarding, and competency development initiatives.
  • Ensure adherence to care coordination and discharge planning processes across all cases.
  • Monitor departmental performance using data analysis and implement sustainable improvement strategies.
  • Collaborate with financial and operational leaders to align departmental goals with organizational objectives.
  • Establish and maintain relationships with clinical and ancillary service leaders.
  • Manage on-call rotations for weekends based on hospital volume and operational needs.
  • Uphold the organization’s Code of Conduct and Mission Statement in all professional activities.

Job Requirements

  • Bachelor of Science in Nursing (BSN) is required; Master’s degree is preferred.
  • Active Registered Nurse (RN) licensure or equivalent social work licensure is required.
  • Minimum of 3 years of acute care hospital case management experience is required.
  • At least 2 years of leadership experience in case management is required.
  • Proven ability to build strategic relationships with physicians and post-acute care stakeholders.
  • Excellent communication, presentation, and motivational skills are essential.
  • Strong adaptability and ability to manage multiple priorities effectively.
  • Self-motivated with a robust work ethic and ability to develop actionable plans independently.
