Overview

Key Responsibilities & Duties
- Conduct thorough physical assessments and provide direct nursing care to program participants.
- Develop individualized care plans and update them based on evolving health needs.
- Supervise and train Licensed Practical Nurses and support staff in nursing protocols.
- Coordinate acute and ongoing health services with clinical personnel and external providers.
- Monitor participant health status and document outcomes within electronic health records.
- Conduct training sessions on medication administration, emergency first aid, and health practices.
- Participate in interdisciplinary meetings to optimize care strategies and service delivery.
- Ensure compliance with OPWDD standards and healthcare regulations at all levels.
Job Requirements
- Associate of Applied Science (AAS) degree in Nursing or a related field.
- Minimum of 1 year of nursing experience, preferably with OPWDD populations.
- Proficiency in electronic health record systems and care coordination practices.
- Knowledge of OPWDD regulations and nursing standards for compliance.
- Ability to train and supervise staff in clinical procedures and health practices effectively.
- Strong communication skills for interdisciplinary collaboration and patient interaction.
- Flexibility to work on-site with occasional on-call availability as needed.
