Job Description

Overview

  • Provide nutritional assessments and care to older adults in an Adult Day Program setting.
  • Collaborate with interdisciplinary teams to ensure comprehensive care plans for participants.
  • Prepare and distribute meals and snacks tailored to participants' dietary needs.
  • Offer diet counseling and education to participants and their families.
  • Develop and revise cycle menus for therapeutic and general diets.
  • Ensure compliance with cultural and religious dietary preferences.
  • Participate in quality improvement programs and audits related to nutrition services.
  • Maintain documentation adhering to best practices and regulatory guidelines.

Key Responsibilities & Duties

  • Conduct initial and quarterly nutritional assessments for program participants.
  • Attend interdisciplinary care plan meetings to provide nutritional insights.
  • Monitor and modify diet prescriptions as per physician orders.
  • Assist in preparing educational materials and in-service training sessions.
  • Collaborate with nursing and food service teams to ensure dietary compliance.
  • Provide follow-up care and continuity in nutritional services.
  • Respect and integrate cultural and religious dietary practices in meal planning.
  • Support quality assurance initiatives and participate in audits.

Job Requirements

  • Registered Dietitian certification with clinical experience preferred.
  • Degree in Nutrition or related field required.
  • Experience working with elderly populations in healthcare or homecare settings.
  • Proficiency in assessing nutritional status and documenting care plans.
  • Strong communication skills for interdisciplinary collaboration.
  • Ability to develop and implement therapeutic and general cycle menus.
  • Knowledge of regulatory guidelines and best practices in nutrition care.
