Job Description

Overview

  • Provide nutritional services for older adults in an Adult Day Program setting.
  • Conduct initial and ongoing nutritional assessments for program participants.
  • Collaborate with interdisciplinary teams to develop and implement care plans.
  • Prepare and distribute meals, snacks, and therapeutic diets as prescribed.
  • Educate clients and staff on nutrition-related topics and best practices.
  • Ensure compliance with cultural, religious, and dietary preferences.
  • Maintain documentation adhering to regulatory guidelines and standards.
  • Support program audits and quality improvement initiatives.

Key Responsibilities & Duties

  • Conduct nutritional assessments and reassessments for program participants.
  • Develop and modify therapeutic diets based on physician orders.
  • Participate in interdisciplinary care plan meetings and conferences.
  • Provide diet counseling and education to clients and their families.
  • Assist in menu planning and implementation for therapeutic diets.
  • Collaborate with nursing and food service staff to ensure dietary compliance.
  • Conduct in-service training sessions on nutrition topics for staff.
  • Monitor and evaluate the effectiveness of nutritional care plans.

Job Requirements

  • Registered Dietitian credentials with clinical experience preferred.
  • Degree in Nutrition or related field required.
  • Experience working with elderly populations in care settings preferred.
  • Proficiency in developing and implementing individualized care plans.
  • Strong communication and collaboration skills.
  • Ability to respect cultural and religious dietary preferences.
  • Knowledge of regulatory guidelines and best practices in nutrition care.
