Overview
- Provide comprehensive psychosocial support to hospice patients and families in various care settings, including inpatient, home care, and nursing homes.
- Collaborate with interdisciplinary teams to develop tailored care plans addressing patient and family needs.
- Assist patients in pre and post-bereavement phases, ensuring emotional and social well-being.
- Act as a liaison between hospice staff and external care providers to ensure seamless service delivery.
- Develop discharge plans for patients transitioning from inpatient facilities to other care settings.
- Maintain accurate clinical documentation and input data proficiently using relevant software tools.
- Work within a supportive team environment to deliver high-quality hospice care services.
- Participate in training and supervision to enhance professional skills and meet licensure requirements.
Key Responsibilities & Duties

Job Requirements
- Master of Social Work degree from an accredited institution.
- LMSW or LCSW licensure in New York State is mandatory.
- Minimum of one year of experience in a healthcare setting; four years preferred.
- Proficiency in Microsoft Word and Excel for clinical documentation.
- Strong organizational, communication, and listening skills.
- Ability to work effectively within an interdisciplinary team environment.
- Experience with hospice or palliative care is highly desirable.
