Job Description

Overview

  • Provide psychosocial support and social work services to hospice patients and families in various settings.
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care and support.
  • Develop and implement social work care plans tailored to patient and family needs.
  • Assist in pre- and post-bereavement phases to support emotional well-being.
  • Work primarily in the field within the Bronx area, requiring travel to patient locations.
  • Maintain compliance with licensure requirements and professional standards.
  • Participate in interdisciplinary team meetings and contribute to care planning discussions.
  • Provide supervision hours for LCSW licensure as applicable.

Key Responsibilities & Duties

  • Develop and manage social work care plans for hospice patients and families.
  • Coordinate discharge plans for patients transitioning from inpatient facilities.
  • Serve as a liaison between hospice staff and inpatient/nursing home teams.
  • Provide bereavement support and counseling to families during hospice service phases.
  • Collaborate with interdisciplinary teams to ensure holistic patient care.
  • Document clinical data and patient interactions accurately and efficiently.
  • Participate in virtual and in-person interviews as part of the hiring process.
  • Provide weekend coverage as required to support patient needs.

Job Requirements

  • Master of Social Work degree from an accredited institution.
  • Minimum of one year of experience in healthcare social work; hospice/palliative care internship experience preferred.
  • Licensed LMSW or LCSW in New York State.
  • Proficiency in Microsoft Word and Excel for documentation purposes.
  • Strong organizational, communication, and listening skills.
  • Ability to work effectively within an interdisciplinary team environment.
  • Willingness to travel within the Bronx area for fieldwork.
