Job Description

Overview

  • Provide telehealth services to patients, focusing on homecare and ancillary needs.
  • Collaborate with healthcare teams to deliver patient-centered care through innovative telemedicine platforms.
  • Diagnose conditions, assess patient requirements, and devise effective treatment strategies.
  • Work on-site during standard business hours, ensuring consistent patient care delivery.
  • Promote professional growth and serve as a clinical resource within the organization.
  • Assist Clinical Access Center staff and perform additional tasks as directed.

Key Responsibilities & Duties

  • Plan and organize patient care according to established protocols and pathways.
  • Collaborate with healthcare teams to develop and implement effective treatment plans.
  • Conduct comprehensive health assessments and order necessary diagnostic tests.
  • Prescribe medications and medical equipment in compliance with practice protocols.
  • Evaluate treatment effectiveness and modify plans to optimize patient outcomes.
  • Provide guidance and support to clinical staff, fostering professional development.
  • Assist in operational tasks within the Clinical Access Center as needed.
  • Perform additional duties as assigned by the supervisor to support organizational goals.

Job Requirements

  • Current registration as a Nurse Practitioner in New York State.
  • Master of Science in Nursing degree from an accredited institution.
  • Certification as an Adult or Family Nurse Practitioner.
  • Minimum of 2 years of experience in a related healthcare role; 4 years preferred.
  • Strong knowledge of telemedicine platforms and homecare/ancillary services.
  • Excellent communication and collaboration skills for effective team integration.
  • Ability to assess, diagnose, and treat patients effectively in a telehealth setting.
