Job Description

Overview

  • Lead pharmacy quality initiatives to achieve corporate STAR goals through medication management and adherence improvement programs.
  • Conduct comprehensive medication reviews and develop care plans to optimize patient outcomes and medication adherence.
  • Collaborate with interdisciplinary teams to resolve medication-related problems and enhance therapy effectiveness.
  • Ensure compliance with CMS standards and regulatory requirements in all medication therapy management activities.
  • Provide patient education on disease states, medication usage, and adherence strategies.
  • Support cost-effective medication interventions and formulary management to reduce unnecessary expenses.
  • Maintain comprehensive knowledge of clinical standards and policies related to pharmacy services.
  • Participate in projects and duties to enhance pharmacy service delivery and patient care.

Key Responsibilities & Duties

  • Conduct patient medication history reviews to identify and resolve barriers to care.
  • Recommend drug therapy changes based on patient interviews and medical records.
  • Provide patient monitoring to optimize outcomes and detect adverse drug reactions.
  • Deliver medication therapy management services and document according to CMS standards.
  • Coordinate medication reconciliation across care continuum to prevent readmissions and adverse reactions.
  • Collaborate with prescribers and care teams to develop patient-specific therapy goals.
  • Ensure compliance with clinical standards and regulatory requirements in pharmacy services.
  • Assist in formulary evaluation and maintenance based on clinical requirements.

Job Requirements

  • Bachelor’s degree in Pharmacy or PharmD from an accredited institution.
  • Current, unrestricted Pharmacist License as per state regulations.
  • Completion of MTM training program by APhA or PowerPak C.E. preferred.
  • PGY-1 residency in community pharmacy or managed care pharmacy preferred.
  • Board certification in Clinical Pharmacy via BPS or NBMTM preferred.
  • Minimum 3 years of experience in managed care pharmacy; 5 years preferred.
  • Proficiency in CMS Comprehensive Medication Reviews and STAR measures.
  • Strong communication, critical thinking, and problem-solving skills.
