Job Description

Overview

  • Provide essential care and support to individuals in a residential setting, enhancing their quality of life.
  • Accompany and advocate for individuals during medical appointments, ensuring their healthcare needs are met.
  • Assist residents with daily activities, including personal care, recreational programs, and household tasks.
  • Document medical appointments and treatments accurately, ensuring communication with relevant staff.
  • Maintain a clean, safe, and organized living environment adhering to health and safety regulations.
  • Develop positive relationships with residents, families, staff, and healthcare providers.
  • Participate in interdisciplinary treatment meetings to implement individualized treatment plans.
  • Complete required training such as SCIP and AMAP, applying knowledge in daily responsibilities.

Key Responsibilities & Duties

  • Provide direct care and support to individuals, ensuring their well-being and safety.
  • Coordinate and attend medical appointments, ensuring timely and safe transportation.
  • Advocate for residents during medical appointments, ensuring appropriate care and follow-up.
  • Support individuals in personal care, recreational activities, and household tasks.
  • Document medical appointments, treatments, and medication administration accurately.
  • Maintain professional relationships with residents, families, and healthcare providers.
  • Ensure compliance with health and safety regulations within the living environment.
  • Participate in treatment meetings, contributing to individualized treatment plans.

Job Requirements

  • High School diploma or GED equivalent is required.
  • Valid Driver's License to drive in New York State is mandatory.
  • One year of experience working with OPWDD populations is preferred.
  • Successful completion of OPWDD Medication Certification Course, CPR, First Aid, and SCIP-R training.
  • Ability to lift 50 pounds and assist individuals with movement.
  • Non-sedentary role requiring standing and walking for extended periods.
  • Availability for various shifts including evenings and overnights.
