Overview
- Provide compassionate care and guidance to patients and families during hospice admissions, ensuring a smooth transition to hospice services.
- Conduct comprehensive initial assessments and develop tailored nursing care plans to address individual patient needs.
- Collaborate with multidisciplinary healthcare teams to deliver seamless and holistic patient care.
- Utilize advanced electronic medical records (EMR) systems for accurate and efficient documentation.
- Educate patients and families on hospice philosophy, services, and advance directives to foster understanding and comfort.
- Participate in a rotating weekend schedule to ensure consistent and reliable patient care.

Key Responsibilities & Duties
- Conduct detailed patient and family counseling to explain hospice services and philosophy.
- Perform thorough physical and pain assessments during initial visits to determine care needs.
- Develop initial nursing and home health aide care plans tailored to patient requirements.
- Obtain necessary consents, authorizations, and medical orders from primary physicians efficiently.
- Assist with referral intake and data entry to streamline admission processes.
- Participate in weekend rotations to provide continuous and reliable patient care.
- Coordinate with multidisciplinary teams to ensure comprehensive and seamless care delivery.
- Maintain precise and timely documentation in EMR systems to uphold care standards.
- Support additional admission procedures and responsibilities as required by the role.
Job Requirements
- Registered Nurse licensed to practice in New York State, with a Bachelor of Science in Nursing (BSN) preferred.
- Minimum of two years of recent bedside nursing experience, with six years preferred.
- Experience in Med-Surg nursing and providing care to terminally ill patients.
- Proficiency in electronic medical records (EMR) systems and effective communication skills.
- Ability to assess patient needs and develop comprehensive care plans accordingly.
- Valid driver’s license, reliable vehicle access, and current insurance required.
- Commitment to hospice philosophy and delivering compassionate care to patients and families.
- Flexibility to work across designated counties and participate in weekend rotations.
