Primary Care Physician

Overview

• Provide patient-centered primary care services, emphasizing holistic health and preventive care strategies.

Key Responsibilities & Duties

• Conduct comprehensive assessments, diagnoses, and treatments for diverse patient populations.
• Develop and implement personalized care plans in collaboration with patients and families.
• Lead and coordinate care teams to ensure seamless healthcare delivery.
• Educate patients on health promotion, disease prevention, and lifestyle modifications.
• Document patient encounters and care plans accurately using electronic medical records.
• Participate in quality improvement initiatives to enhance clinical standards.
• Collaborate with specialists and hospitalists for effective patient transition management.
• Supervise and mentor Advanced Practice Practitioners and other healthcare professionals.

Job Requirements

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
• Board certification in Internal Medicine, Family Medicine, or Geriatrics required.
• Active state medical license or eligibility to obtain one promptly.
• Proficiency in electronic medical records and healthcare technology systems.
• Basic Life Support (BLS) certification required within 90 days of employment.
• Strong commitment to patient-centered care and value-based healthcare practices.
• Excellent communication and leadership skills to foster team collaboration.
• Ability to travel locally for training and occasional regional or national events.
