Key Responsibilities & Duties

  • Provide comprehensive care for patients with acute and chronic conditions, emphasizing preventive measures.
  • Lead and collaborate with care teams to ensure high-quality, coordinated care delivery.
  • Document patient interactions and care plans accurately using organizational systems.
  • Engage with specialists and hospitalists to manage patient care transitions effectively.
  • Participate in clinical meetings and quality improvement initiatives to enhance care standards.
  • Mentor and recruit new providers, contributing to the growth of the medical team.
  • Supervise Advanced Practice Practitioners, ensuring adherence to clinical guidelines.

Job Requirements

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
  • Board certification in Internal Medicine, Family Medicine, or Geriatrics mandatory.
  • Active state medical license or eligibility to obtain one within the required timeframe.
  • Minimum of 1 year of experience in primary care, with 5 years preferred.
  • Proficiency in electronic medical records and documentation systems.
  • Strong commitment to patient-centered care and value-based care principles.
  • Basic Life Support (BLS) certification required within the first 90 days of employment.
  • Ability to travel locally for patient care and nationally for training as needed.
