Key Responsibilities & Duties
- Provide comprehensive care to patients, addressing both acute and chronic conditions effectively.

- Collaborate with hospitalists and care teams for seamless patient transitions and management.

Job Requirements
- Doctor of Medicine (MD) degree with active licensure in the state of employment.
- Board certification or eligibility in Internal Medicine, Family Medicine, or Geriatrics.
- Minimum of 5 years of clinical experience, with 10 years preferred.
- Proficiency in Microsoft Office Suite and healthcare applications.
- Basic Life Support (BLS) certification within 90 days of employment.
- Strong leadership and mentoring skills to effectively guide clinical teams.
- Experience in value-based care and patient-centered healthcare delivery.
- Ability to travel locally and nationally for training and recruitment purposes.
