Overview

Key Responsibilities & Duties
- Conduct patient screenings, examinations, diagnoses, and treatments across various age groups and health conditions.
- Provide evidence-based care that is compassionate, respectful, and tailored to individual patient needs.
- Collaborate with healthcare professionals to ensure seamless and coordinated patient care.
- Make appropriate referrals to internal and external specialists to address patient health needs.
- Participate in case conferences and contribute to quality improvement initiatives within the organization.
- Engage in continuing medical education to stay updated on best practices and advancements in healthcare.
- Document patient interactions and care promptly and accurately using EPIC systems.
- Ensure efficiency and patient satisfaction by adhering to scheduled appointment times and providing timely follow-ups.
Job Requirements
- Doctor of Medicine (MD) with active New York State Certification/Licensure.
- Current NYS Registration and DEA Controlled Substances Registration Certificate.
- Board certified or eligible in Family Medicine, Internal Medicine, or Infectious Disease.
- Minimum of 2 years of primary care experience; 3 years preferred.
- Experience in community-based primary care settings is highly desirable.
- Proficiency in bilingual communication (Spanish/English) is a plus.
- Strong collaboration skills within a team-based care model.
