Job Description

Overview

  • Manage and oversee the Memory Care program, ensuring high-quality services for registrants and caregivers.
  • Coordinate program admissions, attendance, and caregiver support while addressing challenges effectively.
  • Conduct initial and ongoing social assessments for registrants, maintaining thorough documentation.
  • Supervise staff, providing orientation, training, and performance evaluations to ensure program success.
  • Collaborate with transportation staff to ensure efficient and safe services for registrants.
  • Coordinate caregiver support groups and maintain professional competence through seminars and courses.
  • Develop and oversee monthly activity schedules tailored to registrants' needs and preferences.
  • Lead care planning meetings, ensuring person-centered and individualized care plans.
  • Maintain collaborative contacts with external social service agencies, representing the program effectively.

Key Responsibilities & Duties

  • Supervise staff to ensure program needs are met and report to the Administrative Manager.
  • Coordinate all aspects of the Memory Impaired program, including admissions, attendance, and caregiver support.
  • Ensure timely and thorough daily documentation by staff.
  • Provide orientation and training for new staff, interns, and volunteers.
  • Work with transportation staff to optimize pick-up and arrival times.
  • Coordinate and lead caregiver support groups and care planning meetings.
  • Ensure adequate staffing and supplies for daily activities and report gaps to the Administrative Manager.
  • Evaluate registrant health records biannually and at discharge for accuracy and completeness.
  • Respond appropriately to registrant emergencies, enlisting help from relevant staff.

Job Requirements

  • Master of Social Work (MSW) degree or equivalent license/certification.
  • Minimum of 5 years of management experience overseeing clinical teams.
  • At least 5 years of experience in dementia/memory care programs.
  • Certification as a Dementia Care Specialist is preferred.
  • Strong organizational and leadership skills to manage program operations effectively.
  • Ability to evaluate workloads and motivate staff to perform efficiently.
  • Excellent communication skills for interaction with caregivers, staff, and external agencies.
  • Proficiency in maintaining accurate and comprehensive documentation.
  • Commitment to professional development through attending relevant courses and seminars.
