- Provide specialized clinical care to hospice inpatients, addressing their unique needs with expertise.
- Collaborate with interdisciplinary teams to create individualized care plans tailored to patient requirements.
- Educate patients, families, and healthcare providers about hospice services and benefits.
- Ensure meticulous documentation of patient care and services to uphold compliance standards.
- Adapt to flexible working hours, including weekends, to meet dynamic patient and organizational needs.
Key Responsibilities & Duties
- Conduct thorough assessments to determine patient eligibility for hospice or palliative care services.
- Collaborate with hospital personnel to streamline hospice admissions and referrals.
- Coordinate daily care for hospice patients, ensuring adherence to General Inpatient level standards.
- Foster and strengthen professional relationships with hospital partners to support service goals.
- Provide timely responses to referral requests, delivering essential information to healthcare staff.
- Apply regulatory guidelines to accurately determine hospice eligibility criteria.
- Participate actively as an RN member of the interdisciplinary hospice care team.
- Educate patients, families, and healthcare providers on the benefits and scope of hospice services.
Job Requirements
- Associate of Applied Science (AAS) in nursing or a related field.
- Minimum of two years of experience in medical-surgical or community health nursing.
- Preferred experience in hospice or oncology nursing for enhanced patient care expertise.
- Active New York State RN licensure and CPR-BLS certification are mandatory.
- Possession of a valid NYS Driver's license is advantageous.
- Proficiency in Microsoft Office applications for efficient documentation and communication.
- Exceptional communication, organizational, and critical thinking skills are essential.
- Ability to work empathetically and effectively with diverse patient populations.
