Overview

  • Lead the strategic direction and operational management of home health services, ensuring exceptional patient care and service delivery.
  • Collaborate with interdisciplinary teams to design and implement innovative care models tailored to patient needs.

Key Responsibilities & Duties

  • Supervise and coordinate daily operations of home health services, ensuring alignment with organizational goals.
  • Recruit, train, and evaluate nursing staff, promoting a culture of continuous improvement and collaboration.
  • Monitor and enforce adherence to clinical protocols, policies, and regulatory standards.
  • Analyze performance metrics to identify opportunities for improvement and implement corrective actions.
  • Facilitate seamless transitions of care by coordinating with multidisciplinary teams and external partners.
  • Develop and manage budgets, ensuring efficient use of resources and financial sustainability.

Job Requirements

  • Bachelor of Science in Nursing (BSN) with at least five years of relevant experience.
  • Active Registered Nurse License and Basic Life Support certification required.
  • Minimum four years of leadership experience, preferably in home health or hospice settings.
  • Comprehensive knowledge of healthcare regulations, compliance, and quality standards.
  • Demonstrated expertise in managing large teams and fostering collaboration across healthcare systems.
  • Proficiency in operational and financial management within healthcare environments.
  • Strong leadership skills in emergency preparedness and recovery planning.
  • Preferred experience in Northern California healthcare systems and home health management.
