Key Responsibilities & Duties

  • Diagnose and manage acute and chronic conditions in patients, focusing on older adults.
  • Develop personalized care plans tailored to individual patient health goals.
  • Lead and supervise a dedicated care team, fostering collaboration and communication.
  • Coordinate care with hospitalists and specialists to ensure comprehensive treatment.
  • Educate patients on health promotion, disease prevention, and lifestyle modifications.
  • Maintain accurate documentation of patient interactions and care plans using electronic health records.
  • Participate in clinical and administrative meetings to align with organizational objectives.
  • Ensure adherence to medical guidelines and quality standards in all aspects of patient care.

Job Requirements

  • Doctor of Medicine (MD) degree with board certification in Internal Medicine, Family Medicine, or Geriatrics.
  • Active state medical license and DEA certification for prescribing controlled substances.
  • Minimum of 1 year of experience in primary care settings, with 5 years preferred.
  • Proficiency in electronic health record systems and standard office software.
  • Strong leadership and communication skills to guide care teams and patient interactions.
  • Commitment to ongoing professional development and continuous quality improvement.
  • Ability to travel locally for patient care and nationally for training or recruitment.
  • Basic Life Support (BLS) certification required within 90 days of employment.
