Job Description

Overview

  • Deliver compassionate hospice care, addressing urgent needs during after-hours shifts in a hybrid environment.
  • Coordinate with the hospice team to ensure seamless patient support and care plan implementation.
  • Provide education to patients and caregivers on medication management, safety, and symptom control.
  • Offer emotional support and guidance to families during patient transitions and end-of-life care.
  • Communicate effectively with team members to report patient updates and follow-up needs.
  • Adhere to professional nursing standards and regulations in all aspects of care delivery.
  • Work in a dynamic hybrid setting, combining remote coordination and field-based responsibilities.

Key Responsibilities & Duties

  • Respond promptly to triage requests, coordinating with the on-call team and hospice physician.
  • Conduct unscheduled patient visits to address urgent needs during assigned shifts.
  • Educate patients and caregivers on care plan adjustments, medication, and symptom management.
  • Provide emotional support and guidance to families during patient transitions and end-of-life care.
  • Document patient visit outcomes and communicate updates to the hospice team.
  • Ensure compliance with professional nursing standards and organizational protocols.
  • Utilize transportation resources for field visits across assigned areas as required.

Job Requirements

  • Associate of Applied Science (AAS) degree from an accredited School of Nursing.
  • Minimum of 2 years of experience in Medical-Surgical, Community Health, Hospice, or Oncology nursing.
  • Active New York State RN licensure and valid NYS driver’s license preferred.
  • Proficiency in Microsoft Word, Excel, and Windows applications.
  • Commitment to obtaining CHPN certification within one year of hire.
  • Ability to work flexible hybrid shifts, including weekends and after-hours.
  • Strong interpersonal and communication skills for effective patient and team interactions.
