Overview

Key Responsibilities & Duties

• Conduct thorough patient assessments, including health histories and physical examinations.
• Prescribe medications and develop personalized treatment plans tailored to individual patient needs.
• Order, interpret, and analyze diagnostic tests to inform clinical decisions.
• Maintain accurate and detailed medical records for all patient interactions.
• Collaborate with physicians and healthcare professionals to optimize patient outcomes.
• Provide specialized care to Medicaid patients, adhering to certification requirements.
• Utilize Creole language skills to improve communication and patient satisfaction.
• Participate in continuous professional development and contribute to team initiatives.

Job Requirements

• Master of Science in Nursing and certification as a Family Nurse Practitioner (FNP).
• Minimum of 1 year of experience as a Nurse Practitioner; 4 years preferred.
• Medicaid certification and ability to provide care to Medicaid patients.
• Proficiency in Creole language for effective communication with patients.
• Strong diagnostic and treatment planning skills to ensure high-quality care.
• Flexibility to work evening and weekend shifts as required.
• Excellent documentation and communication abilities to support clinical operations.
• Commitment to delivering patient-centered care in a collaborative environment.
