Overview

Key Responsibilities & Duties

• Perform thorough patient assessments and develop detailed care plans in collaboration with physicians and healthcare teams.
• Deliver home health services, including patient admissions, re-certifications, and discharges, ensuring comprehensive care.
• Educate patients and families on preventive care strategies and healthy lifestyle practices for improved well-being.
• Maintain accurate and timely documentation of patient care activities using electronic health record systems.
• Adhere to Department of Health standards and organizational protocols to ensure quality and compliance.
• Collaborate with interdisciplinary teams to provide holistic, patient-centered care and address individual needs.
• Engage in professional development opportunities to stay updated on best practices and enhance nursing expertise.
• Communicate effectively with patients, families, and healthcare professionals to ensure optimal care delivery.

Job Requirements

• Associate of Applied Science (AAS) in Nursing or equivalent educational qualification.
• Valid nursing license and proven experience as a registered nurse in relevant settings.
• Experience in pediatric and acute care environments, preferably in home healthcare.
• Proficiency in completing OASIS assessments for patient care planning and documentation.
• Familiarity with Homecare Homebase (HCHB) software or similar electronic health record systems.
• Reliable transportation and a valid driver’s license for travel to patient locations.
• Strong organizational and communication skills to manage caseloads effectively and ensure quality care.
