Overview

Key Responsibilities & Duties

  • Perform thorough patient assessments and develop detailed care plans in collaboration with physicians and healthcare teams.

Job Requirements

  • Associate of Applied Science (AAS) in Nursing or equivalent educational qualification.
  • Valid nursing license and proven experience as a registered nurse in relevant settings.
  • Experience in pediatric and acute care environments, preferably in home healthcare.
  • Proficiency in completing OASIS assessments for patient care planning and documentation.
  • Familiarity with Homecare Homebase (HCHB) software or similar electronic health record systems.
  • Reliable transportation and a valid driver’s license for travel to patient locations.
  • Strong organizational and communication skills to manage caseloads effectively and ensure quality care.
