Job Description

Overview

  • Provide primary and preventive care to long-term care patients in skilled nursing facilities.
  • Manage residents experiencing acute changes in condition and deliver advanced care planning.
  • Prescribe medications, including controlled substances, in compliance with state regulations.
  • Coordinate treatments and diagnostic testing, ensuring effective follow-up and care plans.
  • Collaborate with nursing home staff and primary care physicians to enhance patient outcomes.
  • Utilize electronic medical records (EMR) to manage patient data and care coordination.
  • Address advanced illnesses and multiple chronic conditions in an interdisciplinary care team setting.

Key Responsibilities & Duties

  • Perform comprehensive assessments and develop individualized care plans for patients.
  • Monitor and manage patients with chronic conditions, ensuring optimal health outcomes.

Job Requirements

  • Board Certification and DEA licensure are mandatory.
  • Experience in adult, family, or geriatric specialties is preferred.
  • Proficiency in managing patients with advanced illnesses and chronic conditions.
  • Familiarity with electronic medical records (EMR) systems.
  • Ability to work collaboratively within an interdisciplinary care team.
  • Psychiatric experience is advantageous but not mandatory.
