Overview

Key Responsibilities & Duties
- Conduct comprehensive assessments and provide primary care to residents.
- Prescribe and manage medications, ensuring compliance with legal standards.
- Collaborate with physicians and care teams to optimize patient health outcomes.
- Perform advanced care planning for patients with chronic conditions.
- Order and interpret diagnostic tests to support clinical decision-making.
- Communicate patient care plans effectively to nursing home staff.
- Provide education and guidance to patients and families regarding care options.
- Utilize electronic medical records (EMR) for accurate documentation and tracking.
- Support the implementation of strategies to improve facility care standards.
Job Requirements
- Doctorate of Nursing Practice (DNP) degree required.
- Current, unrestricted Nurse Practitioner license in the state of practice.
- Board Certification as a Nurse Practitioner.
- DEA licensure for prescribing controlled substances.
- Experience in adult, family, or geriatric specialties preferred.
- Proficiency in managing patients with chronic conditions and advanced illnesses.
- Experience in long-term care settings is advantageous.
- Familiarity with electronic medical records (EMR) systems.
- Psychiatric experience is a plus.
