Job Description

Overview

  • Provide primary and preventative care to patients in skilled nursing facilities.
  • Manage acute changes in patient conditions and chronic care needs effectively.
  • Prescribe medications, including controlled substances, adhering to state regulations.
  • Collaborate with interdisciplinary teams to coordinate comprehensive patient care plans.
  • Utilize advanced care planning to enhance patient outcomes and satisfaction.
  • Engage in diagnostic testing and treatment follow-ups to ensure quality care.
  • Communicate effectively with nursing staff regarding patient conditions and care plans.
  • Support long-term care patients with advanced illnesses and multiple chronic conditions.
  • Contribute to improving CMS ratings through outcome-driven resident care strategies.

Key Responsibilities & Duties

  • Conduct comprehensive assessments and provide primary care to residents.
  • Prescribe and manage medications, ensuring compliance with legal standards.
  • Collaborate with physicians and care teams to optimize patient health outcomes.
  • Perform advanced care planning for patients with chronic conditions.
  • Order and interpret diagnostic tests to support clinical decision-making.
  • Communicate patient care plans effectively to nursing home staff.
  • Provide education and guidance to patients and families regarding care options.
  • Utilize electronic medical records (EMR) for accurate documentation and tracking.
  • Support the implementation of strategies to improve facility care standards.

Job Requirements

  • Doctorate of Nursing Practice (DNP) degree required.
  • Current, unrestricted Nurse Practitioner license in the state of practice.
  • Board Certification as a Nurse Practitioner.
  • DEA licensure for prescribing controlled substances.
  • Experience in adult, family, or geriatric specialties preferred.
  • Proficiency in managing patients with chronic conditions and advanced illnesses.
  • Experience in long-term care settings is advantageous.
  • Familiarity with electronic medical records (EMR) systems.
  • Psychiatric experience is a plus.
