Job Description

Overview

  • Provide primary and preventative care for patients in skilled nursing facilities.
  • Manage residents experiencing acute changes in condition and chronic care needs.
  • Prescribe medications, including controlled substances, adhering to state regulations.
  • Collaborate with interdisciplinary teams to coordinate comprehensive patient care.
  • Utilize electronic medical records (EMR) for patient documentation and care planning.
  • Engage in advanced care planning for patients with complex health conditions.
  • Conduct diagnostic testing and follow-up treatments as necessary.
  • Communicate effectively with nursing home staff regarding patient care plans.

Key Responsibilities & Duties

  • Provide comprehensive primary care and preventative services to long-term care residents.
  • Diagnose and manage acute and chronic health conditions.
  • Prescribe medications and oversee treatment plans in compliance with regulations.
  • Collaborate with physicians and healthcare teams for optimal patient outcomes.

Job Requirements

  • Doctorate of Nursing Practice (DNP) and unrestricted NP license in the state.
  • Board Certification and DEA licensure required.

  • Ability to manage complex health conditions and advanced illnesses.
  • Strong collaboration skills within interdisciplinary care teams.
  • Experience in long-term care settings is highly desirable.
