Overview
- Provide primary and preventive care for patients in skilled nursing facilities.
- Manage residents experiencing acute changes in condition and chronic illnesses.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
- Prescribe medications, including controlled substances, adhering to state regulations.
- Order and interpret diagnostic tests and follow up on treatments.

Key Responsibilities & Duties
- Conduct comprehensive assessments of patients in long-term care settings.

Job Requirements
- Doctorate of Nursing Practice (DNP) with board certification.
- Current, unrestricted Nurse Practitioner license in the state of practice.
- DEA licensure for prescribing controlled substances.
- Clinical experience in adult, family, or geriatric specialties.
- Proficiency in managing patients with advanced illnesses and chronic conditions.
- Experience in long-term care settings and interdisciplinary team collaboration.
- Knowledge of electronic medical records (EMR) systems.
