Overview
- Provide specialized wound care services for long-term care patients in skilled nursing facilities.
- Collaborate with interdisciplinary teams to optimize patient outcomes and care plans.

Key Responsibilities & Duties
- Perform wound assessments and develop individualized treatment plans.
- Prescribe medications and advanced wound care products as needed.
- Collaborate with primary care physicians and nursing staff to ensure comprehensive care.

- Educate patients and caregivers on wound care best practices and preventive measures.
- Utilize electronic medical records (EMR) for documentation and communication.

Job Requirements
- Doctorate of Nursing Practice (DNP) degree required.
- Current, unrestricted Nurse Practitioner license in the state of Ohio.
- Board Certification and DEA licensure preferred.
- Experience in adult, family, or geriatric specialties and wound care management.
- Proficiency in diagnosing and treating various wound types and pressure injuries.
- Familiarity with advanced wound care products and techniques.
- Experience with wound vacs and pressure-relieving devices.
- Ability to work effectively within an interdisciplinary care team.
- Strong preference for wound care certification or willingness to obtain certification.
