Job Description

Overview

  • Provide comprehensive primary and preventive care to patients in skilled nursing facilities.
  • Manage acute changes in patients' conditions and coordinate advanced care planning.
  • Prescribe medications, including controlled substances, in compliance with state regulations.
  • Collaborate with interdisciplinary teams to optimize patient care and outcomes.
  • Utilize electronic medical records (EMR) systems effectively for patient documentation.
  • Engage in chronic care management and advanced illness management for patients.
  • Communicate regularly with nursing home staff regarding patient care plans and updates.
  • Provide wound care services as part of the role's responsibilities.

Key Responsibilities & Duties

  • Deliver primary care services to long-term care patients in skilled nursing facilities.
  • Prescribe and manage medications, ensuring compliance with state and federal laws.
  • Order and interpret diagnostic tests to inform treatment plans.
  • Collaborate with physicians and interdisciplinary teams for patient care coordination.
  • Provide wound care and manage chronic conditions effectively.
  • Maintain accurate patient records using EMR systems.
  • Engage in advanced care planning and chronic disease management.
  • Communicate with nursing staff to address changes in patient conditions.

Job Requirements

  • Doctorate of Nursing Practice (DNP) required.
  • Current, unrestricted Nurse Practitioner license in the state of practice.
  • Board Certification and DEA licensure mandatory.
  • Experience in adult, family, or geriatric specialties preferred.
  • Proficiency in EMR systems and interdisciplinary care coordination.
  • Ability to manage patients with advanced illnesses and chronic conditions.
  • Experience in long-term care settings advantageous.
  • Psychiatric experience is a plus.
