Job Description

Overview

  • Provide compassionate advocacy and support to individuals and families facing complex challenges in a healthcare setting.
  • Conduct psychosocial assessments, develop care plans, and deliver counseling and crisis intervention services.
  • Collaborate with multidisciplinary teams to ensure holistic support and promote patient well-being.
  • Connect clients to community resources and advocate for their needs effectively.

Key Responsibilities & Duties

  • Conduct and document psychosocial assessments of patients to identify their needs.
  • Educate patients and families on rights and encourage participation in care decisions.
  • Provide liaison services and referrals to hospital resources and community agencies.
  • Identify mental health needs and provide counseling and referrals as required.
  • Collaborate with healthcare team members to ensure continuity of patient care.
  • Coordinate placements with various care facilities and services for patient transitions.
  • Serve as a resource for emotional and behavioral issues, referring to community services.
  • Advocate for patients and families, ensuring access to necessary support and resources.

Job Requirements

  • Master’s degree in Social Work from an accredited graduate school.
  • Minimum of three years of clinical social work or allied health practice experience.
  • Strong knowledge of community resources and crisis management techniques.
  • Excellent interpersonal skills and empathy to foster positive change.
  • Ability to collaborate effectively with multidisciplinary healthcare teams.
