Overview
- Provide high-quality nursing care to patients in Westchester, ensuring their speedy recovery and overall well-being.
- Educate patients and families on health prevention and healthy habits to promote long-term wellness.
- Collaborate with healthcare professionals to develop and implement effective care plans tailored to each patient.
- Utilize advanced nursing skills to deliver compassionate and professional care with minimal supervision.
- Ensure compliance with health and safety guidelines and government regulations consistently.
- Participate in comprehensive training programs to enhance skills and knowledge in home healthcare.
- Work on-site, providing visiting home health services to an assigned caseload in Westchester.
Key Responsibilities & Duties
- Report daily to the Clinical Manager, ensuring seamless communication and coordination.
- Perform patient admissions, re-certifications, and discharges, adhering to agency standards.
- Develop and document the Physician Plan of Care within the required timeframe using a tablet.
- Conduct OASIS D1 assessments for admissions, re-certifications, transfers, and discharges.
- Deliver home care services in compliance with Department of Health and agency standards.
- Support the agency’s mission by meeting standards of care, communication, and cultural diversity.
- Provide patient education on health prevention and healthy habits to ensure long-term wellness.
Job Requirements
- Associate of Applied Science (AAS) degree in Nursing or equivalent educational background.
- Valid Nursing license and proven experience as a registered nurse.
- Experience in acute care or home healthcare settings is required.
- Proficiency in completing comprehensive OASIS assessments.
- Pediatric experience and familiarity with Homecare Homebase (HCHB) is a plus.
- Must possess a vehicle and a valid driver’s license for patient visits.
