Social Worker – Hybrid

Overview

• Provide compassionate mental health care in a hybrid work environment, supporting diverse patient populations with tailored interventions.

Key Responsibilities & Duties

• Deliver direct mental health support and coordinate care within primary healthcare settings.
• Perform outreach and management of patient cases to ensure comprehensive care delivery.
• Utilize standardized tools for patient assessments and adhere to behavioral health guidelines.
• Participate in clinical supervision to enhance professional skills and care quality.
• Manage caseloads effectively using reporting tools for performance tracking.
• Provide referrals for additional services such as housing support and substance abuse treatment.
• Collaborate with healthcare teams to design and implement treatment protocols.
• Contribute to organizational quality improvement projects and initiatives.

Job Requirements

• Master’s Degree in Social Work or a related field is required.
• NYS licensure as LMSW, LCSW, PhD, or PsyD is preferred.
• Experience working with diverse populations including HIV, homeless, mentally ill, elderly, and children.
• Proficiency in evidence-based treatments for depression and primary care interventions.
• Familiarity with electronic health records (EHR) technology and MS Office applications.
• Bilingual proficiency in Spanish is preferred.
• Current BCLS and Infection Control certifications are required.
• Strong organizational, interpersonal, and communication skills for effective assignment management.
