Key Responsibilities & Duties
- Deliver direct mental health support and coordinate care within primary healthcare settings.
- Perform outreach and management of patient cases to ensure comprehensive care delivery.
- Utilize standardized tools for patient assessments and adhere to behavioral health guidelines.
- Participate in clinical supervision to enhance professional skills and care quality.

Job Requirements
- Master’s Degree in Social Work or a related field is required.
- NYS licensure as LMSW, LCSW, PhD, or PsyD is preferred.
- Experience working with diverse populations including HIV, homeless, mentally ill, elderly, and children.
- Proficiency in evidence-based treatments for depression and primary care interventions.
- Familiarity with electronic health records (EHR) technology and MS Office applications.
- Bilingual proficiency in Spanish is preferred.
- Current BCLS and Infection Control certifications are required.
- Strong organizational, interpersonal, and communication skills for effective assignment management.
