Key Responsibilities & Duties

• Engage with members to review and update care plans, ensuring they align with evolving needs and preferences.
• Coordinate services with families, healthcare providers, and external agencies to optimize care outcomes.
• Document member interactions and updates promptly, adhering to organizational standards and protocols.
• Participate in team meetings to discuss member progress and strategize improvements in care delivery.
• Conduct clinical reviews and assessments to ensure quality care delivery and compliance with regulations.
• Provide bilingual support to facilitate effective communication with Bengali-speaking members and their families.
• Utilize Excel for documentation, reporting, and data management tasks, ensuring accuracy and efficiency.
• Adhere to organizational policies and procedures to maintain high standards of care management and member satisfaction.

Job Requirements

• Bachelor's degree in a relevant field or equivalent professional experience in healthcare or related areas.
• Minimum of 1 year of experience in Managed Long-Term Care (MLTC) or similar settings preferred.
• Proficiency in Bengali language to support effective communication with members and their families.
• Basic proficiency in Excel for managing documentation and performing data-related tasks.
• Strong understanding of member assessment and clinical review processes in healthcare settings.
• Ability to work remotely while maintaining high levels of productivity and member engagement.
• Excellent communication skills to interact effectively with diverse populations and multidisciplinary teams.
• Familiarity with care management documentation standards and reporting requirements preferred.
