Overview
- Coordinate inpatient hospice services, ensuring compassionate care and seamless transitions for patients and families.
- Develop and maintain collaborative relationships with hospital teams to enhance hospice admissions processes.
- Conduct comprehensive patient assessments to determine hospice eligibility and create tailored care plans.
- Educate patients, families, and healthcare providers about hospice benefits and care options.
- Collaborate with interdisciplinary teams to ensure compliance with care standards and regulations.
- Provide clinical expertise to support patients receiving General Inpatient hospice care.
- Adapt to dynamic healthcare environments to address referral needs and promote hospice services.
- Contribute to enhancing patient outcomes and supporting organizational goals.
Key Responsibilities & Duties
- Strengthen partnerships with hospital teams to facilitate effective hospice care coordination.
- Respond promptly to referrals, ensuring accurate documentation and regulatory compliance.
- Evaluate clinical information to determine hospice eligibility and develop appropriate care plans.
- Participate as the RN member of the Inpatient Hospice Interdisciplinary Team (IDT).
- Coordinate daily care for patients receiving General Inpatient hospice services.
- Educate and support patients, families, and hospital personnel regarding hospice care processes.
- Maintain detailed and accurate documentation of patient care activities and referrals.
- Collaborate with healthcare providers to ensure seamless transitions and continuity of care.
Job Requirements
- Associate of Applied Science (AAS) in Nursing or related field required.
- Minimum of 2 years’ experience in Medical-Surgical or Community Health nursing.
- Preferred 4 years of experience in Hospice or Oncology nursing.
- Active RN licensure in New York State and CPR-BLS certification required.
- Valid NYS Driver's license preferred for travel between hospital locations.
- Proficiency in Microsoft Office applications and strong communication skills.
- Ability to work flexible hours, including weekends, to meet position needs.
- Excellent organizational skills and capability to collaborate within interdisciplinary teams.
