Overview

Key Responsibilities & Duties

• Perform comprehensive nursing evaluations, documenting findings accurately in client records for effective care coordination.
• Coordinate interdisciplinary services to ensure seamless and efficient care delivery for clients.
• Administer prescribed medications and treatments, including injections, with precision and adherence to protocols.
• Monitor client adherence to treatment plans, adjusting strategies to optimize health outcomes and engagement.
• Conduct educational sessions for clients and caregivers on health topics to promote informed care practices.
• Act as a liaison between clients, families, and healthcare providers to facilitate communication and support.
• Participate in quality assurance initiatives to uphold high standards of care and service delivery.
• Engage in advocacy efforts to address systemic health challenges and improve community well-being.

Job Requirements

• Active Registered Nurse license in the State of New York.
• Bachelor of Science in Nursing (BSN) from an accredited institution.
• Minimum of 1 year of nursing experience, with 2 years preferred.
• Proficiency in administering injections and managing complex treatment plans effectively.
• Experience in HIV/AIDS care, substance use, or mental health is advantageous.
• Strong organizational and documentation skills for maintaining accurate client records.
• Bilingual proficiency in Spanish/English is preferred.
• Commitment to advocacy and interdisciplinary care planning to address community health challenges.
