Key Responsibilities & Duties

• Provide care for patients with acute and chronic conditions, ensuring accountability for health outcomes.
• Act as a health coach, guiding patients in setting and achieving health goals.
• Lead and collaborate with multidisciplinary teams to deliver comprehensive patient care.
• Engage with hospitalists and transitional care teams for specialized patient needs.
• Participate in risk management programs and clinical meetings to enhance care quality.
• Supervise Advanced Practice Practitioners, ensuring adherence to high care standards.
• Promote evidence-based practices and continuous improvement in clinical operations.
• Contribute to organizational growth by recruiting patients and healthcare providers.

Job Requirements

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
• Board certification in Internal Medicine, Family Medicine, or Geriatrics mandatory.
• Active state medical license or eligibility to obtain one promptly.
• Basic Life Support (BLS) certification from AMA or American Red Cross required.
• Proficiency in clinical documentation software and Microsoft Office Suite.
• Strong dedication to patient-centered care and service excellence.
• Willingness to travel locally for market needs and nationally for training purposes.
• Fluency in English; additional language skills preferred based on center needs.
