Job Description

Overview

  • Lead and manage a comprehensive Memory Care program, ensuring exceptional services for registrants and their caregivers.
  • Develop and implement individualized care plans tailored to registrants' needs and preferences.
  • Supervise and mentor staff, providing training and evaluations to maintain high program standards.
  • Coordinate admissions and attendance processes, ensuring seamless program operations.
  • Facilitate caregiver support groups, offering guidance and resources to enhance their experience.
  • Collaborate with transportation teams to ensure safe and efficient services for registrants.
  • Maintain professional competence through ongoing education and relevant training opportunities.
  • Represent the program effectively in interactions with external social service agencies.

Key Responsibilities & Duties

  • Supervise program staff, ensuring adherence to operational standards and providing performance feedback.
  • Coordinate Memory Care program activities, including admissions, attendance, and caregiver support initiatives.
  • Ensure accurate and timely documentation of registrant assessments and program activities.
  • Provide orientation and training for new staff, interns, and volunteers to integrate them effectively.
  • Work closely with transportation teams to optimize service schedules and safety protocols.
  • Lead caregiver support groups, fostering a collaborative and supportive environment.
  • Monitor and ensure adequate staffing and resources for daily program operations.
  • Evaluate registrant health records for accuracy and completeness, ensuring compliance with standards.
  • Respond effectively to registrant emergencies, coordinating with relevant staff as needed.

Job Requirements

  • Master of Social Work (MSW) degree or equivalent certification required.
  • Minimum of 5 years of experience managing clinical teams in healthcare settings.
  • Proven expertise in dementia or memory care program management.
  • Certification as a Dementia Care Specialist is preferred.
  • Strong leadership and organizational skills to oversee program operations effectively.
  • Excellent communication skills for engaging with caregivers, staff, and external agencies.
  • Proficiency in maintaining accurate documentation and records.
  • Commitment to ongoing professional development through relevant courses and seminars.
