Overview

Key Responsibilities & Duties

  • Diagnose and manage acute and chronic health conditions to ensure optimal patient outcomes.
  • Develop and implement personalized care plans in collaboration with patients and care teams.
  • Educate patients on preventive care measures and lifestyle modifications to promote wellness.
  • Utilize electronic health records effectively to document patient interactions and care plans.
  • Lead and coordinate care team efforts to ensure seamless service delivery and patient satisfaction.
  • Communicate regularly with specialists and hospitalists to facilitate coordinated care.
  • Participate in quality improvement initiatives and adhere to evidence-based clinical guidelines.
  • Attend training sessions and organizational meetings to stay informed on best practices and goals.

Job Requirements

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
  • Board certification in Internal Medicine, Family Medicine, or Geriatrics is mandatory.
  • Active state medical license and DEA certification for controlled substances.
  • Minimum of 1 year of experience in primary care; 5 years preferred.
  • Proficiency in electronic health records and comfortable utilizing technology in practice.
  • Strong commitment to patient-centered care and value-based healthcare principles.
  • Excellent communication and leadership skills to guide and inspire care teams.
  • Basic Life Support (BLS) certification required within 90 days of employment.
