Overview

  • Deliver compassionate and comprehensive nursing care to hospice patients in a hybrid work setting, ensuring their comfort and dignity.
  • Collaborate with interdisciplinary teams to create and implement personalized care plans tailored to patient needs.
  • Provide education and emotional support to patients and families, fostering understanding and trust.
  • Conduct thorough assessments to identify patient requirements and adjust care plans accordingly.
  • Administer medications and treatments while adhering to established nursing standards and protocols.
  • Document all patient interactions and care activities accurately, ensuring seamless communication with healthcare teams.
  • Participate in team meetings and contribute to continuous improvement initiatives within the hospice care program.
  • Utilize community resources effectively to enhance patient care and support services.

Key Responsibilities & Duties

  • Perform initial and ongoing patient assessments to determine evolving care needs.
  • Develop and implement individualized care plans in collaboration with patients and families.
  • Coordinate with physicians and interdisciplinary teams to ensure cohesive care delivery.
  • Administer prescribed medications and therapeutic procedures, ensuring patient safety and comfort.
  • Provide health education and counseling to patients and their families, addressing concerns and promoting well-being.
  • Accurately document patient care activities and maintain up-to-date clinical records.
  • Supervise and support home health aides, ensuring adherence to established care plans.
  • Participate in case management meetings, sharing insights to optimize patient outcomes.

Job Requirements

  • Bachelor’s degree in nursing from an accredited institution is preferred.
  • Current RN licensure and CPR certification are mandatory.
  • Minimum of 1 year of acute or home care nursing experience; 5 years preferred.
  • Proficiency in nursing skills, problem-solving, and effective communication.
  • Ability to lift and transfer patients and equipment safely.
  • Valid driver’s license and willingness to travel in various weather conditions.
  • Familiarity with computer systems and electronic health records is advantageous.
