Overview

Key Responsibilities & Duties
- Perform initial and ongoing patient assessments to determine evolving care needs.
- Develop and implement individualized care plans in collaboration with patients and families.
- Coordinate with physicians and interdisciplinary teams to ensure cohesive care delivery.
- Administer prescribed medications and therapeutic procedures, ensuring patient safety and comfort.

Job Requirements
- Bachelor’s degree in nursing from an accredited institution is preferred.
- Current RN licensure and CPR certification are mandatory.
- Minimum of 1 year of acute or home care nursing experience; 5 years preferred.
- Proficiency in nursing skills, problem-solving, and effective communication.
- Ability to lift and transfer patients and equipment safely.
- Valid driver’s license and willingness to travel in various weather conditions.
- Familiarity with computer systems and electronic health records is advantageous.
