Overview

  • Collaborate with multidisciplinary teams to enhance discharge planning and post-acute care coordination.
  • Develop and implement innovative strategies to improve care management outcomes and patient satisfaction.
  • Ensure compliance with healthcare regulations and standards, maintaining excellence in care delivery.
  • Monitor performance metrics and drive initiatives to enhance quality and efficiency.
  • Provide leadership and mentorship to staff, fostering professional growth and teamwork.
  • Support community health goals through alignment of care management strategies with population health objectives.
  • Champion continuous improvement in care management practices and patient outcomes.

Key Responsibilities & Duties

  • Supervise and guide the care management team, including nurses and coordinators, ensuring effective operations.
  • Oversee discharge planning processes and collaborate with post-acute care providers for seamless transitions.
  • Manage staffing, scheduling, and payroll to align with patient care needs and organizational goals.
  • Develop and deliver training programs to enhance staff expertise in care management practices.
  • Participate in committees and initiatives to advance care management strategies and community health objectives.
  • Analyze and address customer feedback to ensure satisfaction with care management services.
  • Collaborate with clinical informatics teams to improve documentation and care management processes.
  • Identify opportunities to reduce readmissions and enhance post-discharge services.

Job Requirements

  • Bachelor’s degree in nursing or Master’s degree in social work with LCSW certification required.
  • Minimum of 5 years of acute care hospital or health plan experience for RN candidates.
  • 7 years of relevant experience required for LCSW candidates in applicable settings.
  • Supervisory or management experience in a clinical role is essential.
  • RN license or LCSW certification is mandatory, with CCM or ACM certification within 2 years of hire.
  • Basic Life Support (BLS) certification and annual workplace violence prevention training required.
  • Knowledge of healthcare provider organizations, health plans, and medical management functions.
  • Experience with Medicaid/Medi-Cal patients and government programs preferred.
