Overview
- Deliver exceptional care as a Family Nurse Practitioner specializing in Women's Health and Primary Care.
- Provide comprehensive healthcare services to underserved populations with diverse medical needs.
- Conduct preventive screenings, chronic disease management, and well-woman exams.
- Collaborate with multidisciplinary teams to achieve superior patient outcomes.
- Engage in both clinic-based and in-home patient care for holistic service delivery.
- Apply evidence-based practices to address primary care and gynecological concerns.
- Support patients through pregnancy, menopause, and chronic condition management.
- Ensure accurate and timely documentation within the Electronic Medical Record (EMR) system.
Key Responsibilities & Duties
- Perform comprehensive history and physical exams to assess patient health and risks.
- Provide contraceptive counseling and services, including advanced procedures like IUD insertions.
- Diagnose and treat sexually transmitted infections and offer sexual health education.
- Develop personalized care plans for managing chronic conditions such as diabetes and hypertension.
- Coordinate care with specialists for prenatal and other specialized medical needs.
- Screen for social determinants of health and provide resources for support.
- Deliver accessible care through Tele-Health platforms and remote consultations.
- Ensure adherence to state regulations and clinical practice standards.
Job Requirements
- Master of Science in Nursing with Family Nurse Practitioner certification.
- Active RN licensure and DEA registration in the state of practice.
- Minimum of 1 year of experience in Women's Health and Primary Care; 2 years preferred.
- Proficiency in phlebotomy, immunizations, and office-based medical procedures.
- Experience working with underserved populations and addressing healthcare barriers.
- Fluency in an additional language is highly desirable.
- Strong clinical judgment and collaborative teamwork skills.
- Dedication to health equity and adherence to professional ethical standards.
