Job Description

Overview

  • Deliver exceptional care as a Family Nurse Practitioner specializing in Women's Health and Primary Care.
  • Provide comprehensive healthcare services to underserved populations with diverse medical needs.
  • Conduct preventive screenings, chronic disease management, and well-woman exams.
  • Collaborate with multidisciplinary teams to achieve superior patient outcomes.
  • Engage in both clinic-based and in-home patient care for holistic service delivery.
  • Apply evidence-based practices to address primary care and gynecological concerns.
  • Support patients through pregnancy, menopause, and chronic condition management.
  • Ensure accurate and timely documentation within the Electronic Medical Record (EMR) system.

Key Responsibilities & Duties

  • Perform comprehensive history and physical exams to assess patient health and risks.
  • Provide contraceptive counseling and services, including advanced procedures like IUD insertions.
  • Diagnose and treat sexually transmitted infections and offer sexual health education.
  • Develop personalized care plans for managing chronic conditions such as diabetes and hypertension.
  • Coordinate care with specialists for prenatal and other specialized medical needs.
  • Screen for social determinants of health and provide resources for support.
  • Deliver accessible care through Tele-Health platforms and remote consultations.
  • Ensure adherence to state regulations and clinical practice standards.

Job Requirements

  • Master of Science in Nursing with Family Nurse Practitioner certification.
  • Active RN licensure and DEA registration in the state of practice.
  • Minimum of 1 year of experience in Women's Health and Primary Care; 2 years preferred.
  • Proficiency in phlebotomy, immunizations, and office-based medical procedures.
  • Experience working with underserved populations and addressing healthcare barriers.
  • Fluency in an additional language is highly desirable.
  • Strong clinical judgment and collaborative teamwork skills.
