Overview

Key Responsibilities & Duties
- Administer prescribed medications and treatments to residents, ensuring accuracy and adherence to protocols.
- Monitor residents' health conditions, promptly reporting changes to the Registered Nurse or Clinical Care Manager.
- Maintain accurate documentation of patient care activities and update medical records regularly.
- Assist in developing personalized care plans tailored to residents' unique needs and preferences.
- Provide compassionate emotional support and education to residents and their families.
- Ensure compliance with infection control protocols and uphold safety standards at all times.
- Supervise and guide nursing assistants and support staff to maintain high-quality care delivery.
- Participate in team meetings and contribute ideas to enhance care quality and operational efficiency.
Job Requirements
- Possess a valid Licensed Practical Nurse (LPN) license in the state of New York.
- Graduate from an accredited practical nursing program with relevant certifications.
- Hold Basic Life Support (BCLS) and IV certification as required for the role.
- Have at least one year of experience in long-term care or similar healthcare settings.
- Experience working with geriatric populations is preferred but not mandatory.
- Demonstrate strong communication and interpersonal skills to foster effective collaboration.
- Commit to rotating weekend shifts and adhere to the facility's scheduling requirements.
