Overview

  • Deliver high-quality nursing care to residents in a supportive long-term care setting as a Licensed Practical Nurse (LPN).
  • Collaborate with Registered Nurses and healthcare teams to ensure optimal patient outcomes and satisfaction.
  • Perform charge nurse duties, including supervising and mentoring nursing staff during assigned shifts.
  • Adhere to healthcare regulations and facility policies to maintain compliance and patient safety.

Key Responsibilities & Duties

  • Administer prescribed medications and treatments to residents, ensuring accuracy and adherence to protocols.
  • Monitor residents' health conditions, promptly reporting changes to the Registered Nurse or Clinical Care Manager.
  • Maintain accurate documentation of patient care activities and update medical records regularly.
  • Assist in developing personalized care plans tailored to residents' unique needs and preferences.
  • Provide compassionate emotional support and education to residents and their families.
  • Ensure compliance with infection control protocols and uphold safety standards at all times.
  • Supervise and guide nursing assistants and support staff to maintain high-quality care delivery.
  • Participate in team meetings and contribute ideas to enhance care quality and operational efficiency.

Job Requirements

  • Possess a valid Licensed Practical Nurse (LPN) license in the state of New York.
  • Graduate from an accredited practical nursing program with relevant certifications.
  • Hold Basic Life Support (BCLS) and IV certification as required for the role.
  • Have at least one year of experience in long-term care or similar healthcare settings.
  • Experience working with geriatric populations is preferred but not mandatory.
  • Demonstrate strong communication and interpersonal skills to foster effective collaboration.
  • Commit to rotating weekend shifts and adhere to the facility's scheduling requirements.
  • Ability to work on-site and comply with organizational policies and procedures.
